Every athlete must have a physical and all paperwork signed on PlanetHS before participating in Athletics. Physicals must be dated after April 1, 2021.

PLANET HS INFORMATION
If your child is a returning 8th grader who was in Athletics last year, please log into your parent and student Planet HS accounts and resign all required paperwork. You will also upload a picture of your new physical to your account. If you do not remember your password, please try resetting it in order to log on.
If your child is a 7th grader or is an 8th grader who is new to Athletics, the following steps apply to you. Planet HS is where all CSISD Athletic programs must house their state required UIL paperwork. Below are step-by-step directions on completing your Planet HS accounts/paperwork. It may be helpful to complete these steps using two different devices (one for the parent and one for the student). Also, the following link shares a very helpful video on creating/linking these two accounts. STEP 1: Student must create a Planet HS account at www.planeths.com
STEP 2: Once the student account is created, go to the Link Accounts tab near the top of the page on the blue bar. Type in the parent email address or phone number (whichever you will use for the parent account). Planet HS will then send an invite to this email address or phone number
STEP 3: Parent must create a Planet HS account at www.planeths.com . Be sure to use the same email address or phone number that you sent the invite to in Step 2 above.
STEP 4: Once logged in to the parent account, the invitation to link accounts will appear. Click ACCEPT
STEP 5: Go to the Athletic Forms tab on the blue bar. Sign all required paperwork on BOTH the parent and student accounts
STEP 6: Upload a picture of the completed physical form that was filled out by your doctor to the link called UIL - Pre-Participation Physical Examination. The physical must be dated after April 1, 2021 and must be signed by the doctor, parent, and student.
Once all paperwork is signed and completed, the status should say “complete” in green letters. If the status says “pending staff approval,” that means you have submitted your medical history and/or physical, but our Athletic Trainer must go in to approve the paperwork. Please allow 1-2 days for our trainer to approve the paperwork.
All of the Planet HS paperwork must be completed before an athlete may be issued Athletic clothes or participate in any physical activity. Again, this paperwork is required by law and is meant to keep your athlete safe as well as give the CSMS coaches adequate information in case of an emergency situation.
